About US
About
As a medical social worker, with over 20 years of experience, I have witnessed older adults that are single or living alone, some with mild to moderate dementia, repeatedly hospitalized. Working closely with these patients I noticed that they all in common not qualifying for Medicare, therefore unaware of how to secure private home care services, no family nearby or simple no family to advocate, or coordinate their care.
Mission
The IAM Care Partners mission is to enrich and enhance the quality of life for older adults through compassionate care coordination. At IAM Care Partners we provide services to adults diagnosed with mild to moderate Alzheimer’s, dementia, diabetes, or hypertension that need care coordination.
